Enroll a new patient in your SMARTVIEW™.
6.3.1. Patient Information
6.3.1.1. For French clinics only
When the National Health Identity is activated for the clinic, it is possible to enroll a patient by using the clinic identity repository.
Enter the search criteria below, then click on Search INS:
- INS Number
- Fill in the 15 digits of the patient INS number.
or
- Last name
- Enter at least the first 3 letters of the patient’s last name
- First name
- Enter at least the first 3 letters of the patient’s first name
- Date of birth
- Enter the patient’s date of birth
Based on the given search criteria, the interrogation of the clinic identity repository may give no results, one patient or more matching patients. The list of matching patients is displayed in a table indicating Last name, First name, Date of birth, Gender, INS number and Address; all retrieved from the repository.
Select the patient you are looking from the list, then click Confirm. Patient information from the identity repository will be used to fill the user profile. In case this is not the matching patient, another search can be done by clicking on the Modify button.
The following National Health Identity criteria are presented on the screen:
- INS Number
- The patient National Health Identity number and type
- Gender
- The patient’s gender
- Place of birth Code
- The place of birth INSEE code
- Birth Last name
- The patient last name at birth
- First name
- the patient’s first name
- First names
- the patient’s first names
For clinics where the National Health Identity is not activated, or if the patient could not be found in the clinic identity repository, it is also possible to enroll a patient by clicking on ‘Enroll without INS’. In this case, no information will be extracted from the identity repository and the patient identification fields need to be completed manually.
- NIR Number
- Social security number (NIR). Enter the 15 digits of the NIR
6.3.1.2. For clinics in all countries (including France)
- * First Name
- Required field. Enter the patient's first name. Up to 50 characters are allowed. This field is automatically filled in for French patient registered with INS.
- * Last Name
- Required field. Enter the patient's last name. Up to 50 characters are allowed. This field is automatically filled in for French patient registered with INS.
- * Date of Birth
- Required field. Select the patient's birth date from the pop-up calendar. This field is automatically filled in for French patient registered with INS.
- Patient ID:
- You have the option to enter the Patient ID Number that you use in your local records system to allow you to search for the patient as you would on your system.
6.3.2. Device Information
- *Device Serial Number
- Required field. Enter the device serial number and click Lookup to validate the number and display the model and type on the page.
- *Implant Date
- Required field. Select the patient's implant date from the pop-up calendar.
- WARNING
- The serial number of the defibrillator is used to attribute the data transmitted from the implanted device to the patient name. A wrong serial number can lead to lost information or misleading information. The look-up feature provides an automatic check that the serial number corresponds to an existing model of MicroPort defibrillators.
6.3.3. Contact Information
- *Address
- Required field. Enter up to 128 characters. This address will be used to deliver the monitoring device to the patient (when applicable).
- *Country
- Required field. Select the patient's country from the drop-down menu.
- *State/Province
- Required field. Select the patient's state or province from the drop-down menu.
- *City
- Required field. Enter up to 50 characters.
- *Zip/Postal Code
- Required field. Enter the patient's zip/postal code up to 10 numbers.
- *Phone Number
- Required field. Enter the phone number according to the country format. You may use spaces, periods, plus signs, parentheses and dashes.
- France format: nn.nn.nn.nn.nn
- Germany format: nn nnn nnn nnn
- Italy format: nn nnnnn nnn
- Spain format: nnn.nnn.nnn
- United Kingdom format: nnnnn nnnnnn

- Select the type of phone number from the drop-down menu: Home, Mobile
- Email
- Enter the email address for this patient. Up to 256 characters are allowed. You may use "@" and "." along with letters and numbers.
6.3.4. Alternate Contacts
Enter additional phone numbers for relatives and friends you may contact for this patient.
Select the type of contact: Spouse, Parent, Sibling, Friend
6.3.5. Physician Information
- Primary Physician
- Select the primary physician for this patient from the drop-down menu.
- Add Referral Physician
- Select a referral physician for this patient from the drop-down menu.
- You may add more than one referral by clicking the Add Referral button.
- If you want to remove a referral from the list, click the Remove button.
6.3.6. Comments
Enter comments about this patient's condition and treatment. Up to 500 characters are allowed.
6.3.7. Save
- Consent
- Select the checkbox to indicate the patient's agreement to participate in remote monitoring and have their records stored electronically.
- Cancel
- Clicking the Cancel button will clear all fields on the page.
- Save and Go to List
- Clicking Save and Go to List will save the patient record and take you to the All Patients Search page, updated with the new patient record.
- Save and Add Another
- Clicking Save and Add Another will save the patient record and refresh this page with clear fields for you to add another patient.
- Save and Customize Profile
- Clicking Save and Customize Profile will save the patient record and take you to the patient's Alert Settings page for you to begin changing the patient's alert settings from the default clinic settings. From there, you can access other patient settings that you can modify such as follow up schedule.
